    Responsibilities

    • Plan and manage managed care services for elder’s who resident in the transitional care neighborhood
    • Perform/mange clinical updates by required time for various managed care organizations
    • Establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions; Includes the patient, the family and the physician in the planning process
    • Initiates appropriate preventive and rehabilitative nursing procedures as needed
    • Counsels the patient and family in meeting nursing and social service related needs
    • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient
    • Communicates with community health related persons to coordinate the care plan prior to discharge
    • Participates in on-call duties as defined by the on-call policy
    • Ensures that arrangements for equipment and other necessary items and services are available upon discharge

    Qualifications

    • Graduate of an accredited nursing program, Baccalaureate degree in nursing or related field, preferred
    • Current license as a Registered/Licensed Practical Nurse or Licensed Social Worker in the state of Ohio
    • Possesses and maintains current CPR certification
    • Minimum of 1 year experience
    • Experience in skilled nursing care preferred
