  • Overview

    In keeping with our United Methodist tradition, we seek to enhance the quality of life and holistic growth of older persons by providing high quality service in a cheerful, timely manner and by communicating, coordinating, and cooperating with others to attain goals. The Director of AL & Wellness is responsible for creating an atmosphere that provides for continuity of quality care and quality of life for all residents in the Assisted Living and Independent Living areas 24 hours per day. This is accomplished by providing clear direction and expectations to all partners about the philosophy of the area in which they work.

     

     

    Responsibilities

    • Oversee independent living wellness services, maintaining an 85% productivity rate
    • Interview, select, on-board, train, assign, and develop partners.
    • Schedule CPR and First Aid training for partners; track attendance to ensure compliance.
    • Complete Wellness partner 90 Day and Annual Evaluations.
    • Assist with partner scheduling and staff partners according to caseload.
    • Set productivity expectations and hold partners accountable for meeting them.
    • Provide work assignments to nurses and aides.
      • Ensure partners complete assignments as scheduled.
    • Provide wellness services as indicated.
    • Conduct new resident assessments.
    • Attend resident care conference meetings.
    • Update incident reports to include interventions, write progress note reflecting interventions, enact interventions, and sign incident report.
    • Coordinate transitions in care from independent living to assisted living and long-term care.
    • Attend Daily Morning Leadership Team Meeting at 10:00 AM.
    • Attend Daily Morning Health Services Leadership Team Meeting at 9:30 AM.
      • Report, including details and analysis, any unplanned hospitalizations or ED visits.
      • Report, including details, analysis, and interventions, any falls.
      • Review progress notes and report relevant information to the Team.
    • Conduct Daily Partner Huddle to relay pertinent, real-time information to staff.
    • Meet with Director of Health Services weekly to establish common goals and evaluate progress toward set objectives.
    • Complete Shepherd visits weekly.
    • Serve as On-Call Nurse one week per month.
    • Provide Monthly Wellness Checks for independent living residents.
    • Conduct periodic staff meetings to receive feedback from partners.
    • Conduct periodic evening, night, and weekend visits to stay connected to partners who work at these times.
    • Provide support to the Assisted Living Coordinator, by completing various assisted-living related tasks, as assigned.
    • Oversee assisted living operations in the absence of the Assisted Living Coordinator.
    • Communicate and build positive relationships with residents and family members. 
    • Assist in the referral management process.

    Qualifications

    Qualifications:
    Education: Current RN license
    Experience: Successful candidate will show strong organizational skills and system development, demonstrate effective communication, provide exceptional customer service and a commitment to quality care. The candidate will also need to demonstrate alignment with OPV mission and values. Previous experience in a continuing care community preferred.

    Required experience:

    • Healthcare experience preferred: 2 years
