Provides physicians ordered skilled nursing care to patients in their homes under the supervision of a Registered Nurse. Adheres to the plan of care and interim physicians' orders. Provides care and/ or services in accordance with professional standards, applicable laws and regulations and Agency policies and procedures.


  • Provides skilled nursing care within the nursing scope of practice, the standard of care, and as ordered on the plan of care and in compliance with physician orders
  • Is supervised by the RN in the patient's home with or without the LPN present
  • Documents skilled care and/or services provided in compliance with federal and state laws and regulations, and Agency policies and procedures
  • Participates in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continues care, to assure that patient care needs are met
  • Promotes and provided patient/family/caregiver education
  • Knowledgeable of federal, state regulations and nurse practice acts
  • Communicates with all members of health care team, including physician appropriately and in a timely manner
  • Notifies physicians and RN/ Director of Clinical Services of any changes in patient conditions and the need to modify the plan of care
  • Participates in Agency's Quality Improvement program
  • Attends Agency meetings and in-service programs as required
  • Obtain the appropriate number of CEUs to maintain re-licensure status
  • Maintains confidentiality of patient information and Agency information at all times




  • Graduate of an accredited school of nursing
  • Current state licensure as a Licensed Practical Nurse
  • Minimum of two (2) years experience in a home health setting
  • Current CPR card
  • Current health certificate/physician examination and TB testing results
  • Valid drivers license and reliable automobile
  • Proof of current automobile insurance
  • Able to  communicate effectively in English, both verbally and in writing
